- Outside the test center: Coordinate directly with your instructor to ensure accommodations are applied to the online exam platform. No request form is needed.
Late requests must be made in person at SEC 205 during office hours (Monday-Friday, 8:00 AM – 5:00 PM). Late requests cannot be accepted on the same day, and all late requests must be submitted at least 48 hours before the scheduled exam.

If you encounter any issues (e.g., incorrect extended time, need to clarify exam instructions), speak with the Proctor on Duty. They can contact your professor or assist with the issue.
It’s recommended to review your syllabus early in the semester and submit test requests for all exams and quizzes you plan to take in the test center. Request must be made at least 6 days prior to each exam.
