Penn State Harrisburg students, staff and faculty can use the following form to provide feedback and/or report concerns to the Office of the Chancellor about matters affecting the Penn State Harrisburg community. Login is required, though submissions are anonymous.
As a reminder, anyone can report acts of bias or intolerance anonymously through the University’s Report Bias webpage. Students may also submit a report by calling the 24-hour hotline at 800-560-1637. For behavior that you believe may violate the University’s Student Code of Conduct, contact Student Accountability and Conflict Resolution.
For bias or intolerance incidents where no student is involved, faculty and staff should contact the Office of Equal Opportunity and Access to file an online complaint or call 814-863-0471.
