| Submit Health History Form | Can be found at the University Health Services' site |
| Change your e-mail forwarding address | Can be completed at ITS Secure Server. |

Student Annual Checklist
- Review current I-20/DS-2019 -- Extend temporary stay (if necessary)
- Review passport -- Extend passport (if necessary)
- File income tax returns (Between January 1 and April 15)
- Report any changes in enrollment or major
- Purchase health insurance or file waiver at the beginning of each fall semester
Before Traveling Outside the United States
- Check VISA validity
- Get I-20 or DS-2019 signed from the International Student Office
- Contact International Student Office
