This form is to be used to request use of the Penn State Harrisburg Kulkarni Theatre. Please complete the form before calling to request the use of the facility. Provide as much information as possible on this form.
After your request is submitted, the Arts Manager will call or email you to discuss the request. If the event is approved, you will receive a fully signed request form via email or campus mail.
Note: Only a fully signed form indicates a confirmed reservation.
