Submission Information

If you are interested in trying out for any of our varsity athletic programs, you must first complete the five necessary "Required Physical Forms" found below. Print, complete and return forms to: Penn State Harrisburg Athletics ATTN: Pre-Participation Forms 777 West Harrisburg Pike Middletown, PA 17057 These forms are required before any tryout, practice, or competition process begins. If you have any questions regarding preliminary requirements, please contact the athletic department at 717-948-6744.

Required Physical Forms

All students trying out and/or participating on a varsity athletic team must complete and sign the forms below:

1. Participation Agreement Form (PDF)
2. Medical Insurance Form (PDF)
3. Sport Medicine Information Form (PDF)

In addition to the above forms, the following two documents must be completed and signed by your family physician or the Penn State Harrisburg team physician performing your physical exam.
